All new students are required to submit proof of immunization against measles, mumps, and rubella. New students must have received the meningococcal meningitis immunization within the last 10 years of their first date of enrollment and report the date on which it was received or submit an acknowledgement of the risks and a signed refusal to receive the immunization. Please complete Part One and Part Two of the Immunization Requirement Form and have your healthcare provider complete Part Three. Mail or hand deliver the completed form by December 1, 2013 to:

Any student not in compliance will not be permitted to register or attend classes. If you were accepted after the December 1 deadline, your completed form must be returned within 30 days of the start of the semester.
