Dear Students,

We wanted to inform you that for those students who submitted any request for academic (non-housing) accommodation letters between August 31-September 8th and have NOT received the letters, please resubmit your request AND contact ODS immediately.  Many students who requested letters during this time were able to downloaded the pdf of their request; however, due to a formatting change, there was an additional submit button at the bottom of the page that many did not see and did not click to finalize the submission.   This means that even though you were able to download your completed form as a PDF, we did not receive the form and therefore were not able to write the accommodation letters.  Thank you to the students who contacted us and alerted us to the problem. 

The situation has been resolved and all of the forms are functioning properly.  Again, if you believe you were one of the affected students, please resubmit the request AND contact us immediately.  We will expedite your request.

Registering with ODS

If you are registering with the Office of Disability Services for the first time, you must call our office or stop by to arrange a disability screening appointment.  You will not be able to receive your accommodations if we do not meet with you to review the policies and procedures of accessing accommodations (applies to housing AND academic accommodations).  To make an appointment or if you have any questions please call Disability Services at the New York City Counseling Center: (212) 346-1526. Please submit your disability documentation by mailing or faxing it prior to your appointment (see below for contact information).  

General Academic Accommodation Requests:

If you are a student in need of academic accommodations and you DO NOT already have documentation from your medical provider, your medical provider may submit a letter explaining your disability and your need for accommodations and/or submit the form below.  If you are request housing accommodations, your provider must complete the form directly above, as it pertains specifically to housing accommodations.  You will need to schedule a disability screening appointment by calling the Counseling Center to review your academic accommodation request(s). Please feel free to contact an ODS staff member at (212) 346-1526 if you have any questions.

Accommodation Letter Request & Update Forms

Students who are currently registered with ODS must request accommodation letters via the online forms.  If you are not registered with ODS, you cannot submit these forms.  You will first need to register with the office.  ODS strongly recommends submitting these forms within the first two weeks of the semester.

Testing Accommodation Request Form (TARF)

Students must submit a TARF for EACH exam or quiz they wish to schedule with ODS. It is the student’s responsibility to return the form FULLY COMPLETED 7 days prior to the exam date and 10 days prior to the final exam date.

TARF Submission Options:

1.      Bringing the form to ODS, located within the Counseling Center @ 156 William St, 8th floor

2.      Scanning and emailing the form to the Accommodations Coordinator, Kate Sullivan, csullivan2@pace.edu or accommodatetesting@pace.edu

3.      Faxing the form to the Counseling Center: 212- 346-1530

Disability Housing Requests:

If you would like to request disability housing accommodations, you must submit disability documentation from a qualified medical provider.  The provider can submit a letter or the 'Professional Disability Assessment Questionnaire, which is included in the packet. Either type of documentation may be faxed (914-989-8047 or 212-346-1530), emailed to the Assistant Director, or mailed to the office, along with the Housing Accommodation Request Packet.  Your request will be reviewed upon receipt. You must also schedule a disability screening appointment by calling the Counseling Center to review your housing accommodation request.  Please feel free to contact the Assistant Director of Disability Services at (212) 346-1526 if you have any questions.  For additional information, please refer to the First Appointment Registration section.  

**Please note ODS recommends that students submit the Housing Accommodation Request Packet 3 weeks before the housing selection to allow time for the Assistant Director to review disability documentation.**
