New York City Disability Services Forms

If you are registering with the Office of Disability Services for the first time, you must also call our office or stop by to arrange a disability screening appointment.  You will not receive your accommodations if we do not speak to you in person or hear from you over the phone.  To make an appointment or if you have any questions please call Disability Services at the New York City counseling center: (212) 346-1526.

The first appointment forms enable students with disabilities to print out forms needed for the disability screening appointment.  These forms can be mailed or faxed back to the Office of Disability Services (see information below), or brought to the disability screening appointment.  The forms can also be filled out in our offices immediately prior to the disability screening appointment in lieu of printing in advance.

Disability Housing Request Form:
