You can download the I-20 application form in the Adobe Acrobat Reader format. For anyone who would like to apply for housing, you can download the Housing Application Form below. If you have a sponsor, fill in the Affidavit of Support. Print one copy for each sponsor. Mail the ELI Application, the $60.00 application fee, the Affidavit of Support (if you have a sponsor) and an original bank letter supporting these documents to the address provided on the Application.
