20011-2012 Student Health Insurance

Dear Pace Student:

The University, in cooperation with United Healthcare, Hagedorn & Company, and the Allen J. Flood Companies, Inc., has designed a mandatory/waiver Student Accident and Sickness Insurance Plan for domestic students attending Pace University during the academic year 2011 – 2012. The plan is underwritten by UnitedHealthcare Insurance Company of New York.

Several coverage features of the program have been improved this year for the benefit of students.  The Maximum Benefit Amount has been increased from $25,000 to $100,000 for each Injury or Sickness.

The certificate of coverage (brochure) provides details of the plan, including benefits, exclusions, any reductions and limitations, and the terms under which your coverage may be continued in force. The 2011-2012 STUDENT ACCIDENT AND SICKNESS INSURANCE PLAN FOR DOMESTIC STUDENTS ATTENDING PACE UNIVERSITY certificate (brochure), may be found at http://www.pace.edu/health-insurance/sites/pace.edu.health-insurance/files/Files/Pace%20University%20Domestic%20Brochure.pdf, and may be downloaded and printed to your personal computer/printer, along with enrollment and claim forms and Insurance ID Card.   Some certificates (brochures) will be available in the Offices of the Deans for Students and University Health Care (UHC).

 

Please note that the line item fee Injury/Sickness Insurance  will automatically be included on your student statement if you are a full-time undergraduate enrolled for 12 (twelve) credits or more, or a graduate level student enrolled for 9 (nine) credits or more.   For students enrolling in September 2011, the annual cost is $895; in January 2012 the eight month cost is $566; in Summer Session I, 2012 the cost is $192; and in Summer Session II, 2012, the cost is $82.

The deadline for requesting a waiver for the Fall Term is September 22, 2011 for the annual coverage; February 4, 2012 for the Spring term coverage; June 13, 2012 for the Summer Session I Term coverage; and July 29, 2012 for the Summer Session II Term coverage.  Students who register after the first day of classes will be permitted to submit the waiver request within seven (7) calendar days of the date of their registration.  Requests for waivers received after these dates will not be honored, so please file early.  You only need to waive once for the academic year.  If you waive in the Fall Term, you do not need to waive for the Spring or Summer Terms.

Providing false information on an on-line waiver or letting existing coverage lapse without informing the Office of Student Assistance (OSA) may result in a charge for Pace Insurance coverage to the student’s account and/or disciplinary action.  The University or its agent(s) reserves the right to verify information regarding alternate coverage provided by the student as part of his/her on-line request to waive out of the Pace Insurance Plan.

Any part-time student registered for six (6) or more credits may purchase the Pace Insurance Plan.  All students enrolled in the Pace Insurance Plan may also enroll a spouse, domestic partner, and/or child (children).  Please see the Part-time Student and Dependent Enrollment Form for details.  These Enrollment Forms and the Claim Form are available at the Plan Administrator’s website at www.uhcsr.com/pace.
