HEALTH
PLAN ENROLLMENT CONSIDERATIONS
During
this enrollment period, you may elect to remain with your current coverage,
change health and dental plans and/or add or drop dependent coverage.
Please keep in mind that your elections will remain in effect for the entire
2003 calendar year, unless there is a qualifying change in family status.

- You
can select from three coverage levels: individual, employee + one or family
for either your medical and/or dental plans.

Domestic partners and the partner’s natural or legally adopted children
can be enrolled in the medical and/or dental plans once the “Statement
of Domestic Partnership” is completed and approved. All contributions
for additional medical and dental coverage for a domestic partner and the
partner’s dependent children will be deducted on an after-tax basis.

You can add or drop dependents, to your coverage whenever you have a qualifying
change in your family status. See “Changes in Family Status” in
this publication for further information.
- You
can contribute to a Health Care Flexible Spending Account to pay for medical
and dental expenses that are not reimbursable by your plan. See “Flexible
Spending Accounts” in this publication for more information.

If your spouse or domestic partner has medical or dental coverage under his
or her employer's plan, you should consider that coverage as you decide on
your medical and/or dental coverage through Pace University.

For the medical or dental plan, dependent children are covered through age
19. Coverage will continue for dependent children with full time student status
at a higher education institution until the end of the month in which they
turn 25 for medical or age 23 for dental.

If you are covering a dependent child who attends college out of the network
area, he or she will need to choose a primary care physician near your place
of residence for routine care. If a medical emergency occurs at college, he
or she will still receive network benefits.
