As much as we love students visiting our offices, we have placed our forms online to save you a trip to OGAASD! Whether you are applying for transfer credit or changing your major, simply download and print the appropriate form. Once you have completed the form (and have attached the required materials when necessary), you may drop it off personally, mail it to us, or fax it in.

All requests are processed in the order in which they are received and we receive a lot of requests, so your patience is appreciated. It is best to submit your request long before you need the information processed in order to avoid any undue stress.

Instructions to fill the Request for Graduate Change of Major/Program form

The office of Graduate Academic Advisement & Student Development is responsible for processing Change of Major/Program requests that involve the completion of a new Program Analysis Sheet. Those requests include:

  • All changes involving Accounting and/or Taxation programs
  • All changes from an MBA to an MS degree program
  • All changes from an MS to an MBA degree program
If your request matches one of the options listed above, submit your Change of Major/Program form to the Office of Graduate Academic Advisement & Student Development.

If your request does not match one of the options listed above, submit your Change of Major/Program form to the Students Accounts & Register Services office.

EXCEPTION: ONLY during your first semester of enrollment, your Change of Major/Program form should be submitted to the Graduate Admission office.
