The University, in cooperation with US Fire Insurance Company, Hagedorn & Company, and the Allen J. Flood Companies, Inc., has designed a mandatory/waiver Student Accident and Sickness Insurance Plan for domestic students attending Pace University during the academic year 2010-2011 (to waive see below).

IMPORTANT NOTICE

Pace Law School requires all students enrolled in the full-time program to carry health insurance. If you have existing coverage you may waive the Pace student health insurance for the Fall Term by September 23, 2010, and again by February 8, 2011 for the Spring Term coverage. To waive see below.

The 2010-2011 Pace University Student Accident and Sickness Insurance Plan for Domestic Students brochure, describing the Terms, coverage, exclusions, and premiums may be found at www.pace.edu/healthinsurance, and may be downloaded and printed to your personal computer/printer, along with enrollment and claim forms and Insurance ID Card. Some brochures will be available in the Offices of the Deans for Students and University Health Care (UHC). Please read carefully as certain benefits have been changed in the program since 2009-2010.

Please note that the Accident/Sickness Insurance fee will automatically be included on your student statement if you are a full-time student. For students enrolling in September 2010, the annual cost is $764; in January 2011, the eight month cost is $512; in Summer Session I, 2011 the three month cost is$252; and in Summer Session II, 2011, the six week cost is $131.

Part-time students will not be automatically enrolled and must enroll using this form. The annual cost is $1,016.

The deadline for requesting a waiver for the Fall Term is September 23, 2010 for the annual coverage; February 8, 2011 for the Spring Term coverage; June 15, 2011 for the Summer Session I Term coverage; and July 28, 2011 for the Summer Session II Term coverage. Students who register after the first day of classes will be permitted to submit the waiver request within seven (7) calendar days of the date of their registration. Requests for waivers received after these dates will not be honored, so please file early. You only need to waive once for the academic year. If you waive in the Fall Term, you do not need to waive for the Spring or Summer Terms.

Providing false information on an on-line waiver or letting existing coverage lapse without informing the Office of Student Assistance (OSA) may result in a charge for Pace Insurance coverage to the student’s account and/or disciplinary action. The University or its agent(s) reserves the right to verify information regarding alternate coverage provided by the student as part of his/her on-line request to waive out of the Pace Insurance Plan.

Any part-time student registered for six (6) or more credits may voluntarily choose to purchase the Pace Insurance Plan. All students enrolled in the Pace Insurance Plan may also enroll a spouse, domestic partner, and/or child (children). Please see the Part-time Student and Dependent Enrollment Form for details. These Enrollment Forms and the Claim Form are available at the Program Administrator’s website at www.ajfusa.com.

IMPORTANT NOTE

TO WAIVE: If you have existing Accident and Sickness Insurance coverage under another policy (self, parent, spouse, etc.) you may waive the mandatory Pace University coverage. The only way for you to do this is via the Internet, using the Pace University Web site at /HealthInsurance. Using Internet Explorer Version 5.5 or better, select the My Pace link in the upper right column and login using your Pace IT Systems User Name and password. There are instructions on “How to Log In” and a reference to Help resources if you have any difficulty. After successfully logging into My Pace, click on the Students tab and find the Health Insurance Waiver link. Click the link and follow the directions. After entering your data, print a copy for your records and then click submit. A confirmation screen will appear. Print a copy of the confirmation screen for your records. This will become an important supporting document confirming that you successfully waived out prior to the deadline date should the premium remain on your account beyond the deadline date. You may view your statement on-line using the Registration link on the My Pace page. Students who do not have access to a computer off campus may use the computers in the Campus Libraries and Computer Resource Centers.
