Identity Confirmation Verification Requirement

Federal Identity Verification (V4 and V5)

To complete identity verification for federal financial aid, students must present an unexpired, valid, government-issued photo identification. This documentation can be submitted in person at the Financial Aid desk in your home campus Solution Center or by mailing a notarized copy of your government-issued photo identification to your home campus’ Financial Aid Office. Financial aid disbursement cannot be completed until the identity verification requirements are received and processed. If these two options are not feasible, please contact the Financial Aid Office to discuss the possibility of alternative arrangements.

How to submit your Government-Issued ID for Identity Verification

  • You must appear in person at your home-campus Financial Aid Office to verify your identity by presenting a valid, government-issued photo identification (ID), such as, but not limited to, a driver's license, other state-issued ID, or passport. The institution will maintain a copy of your photo ID that is annotated with the date it was received and the name of the official at the institution authorized to collect your ID. If in-person submission is required, you may incur travel or other related costs.

  • If you are unable to appear in person to verify your identity, you must provide a notarized copy of your government issued ID. The original notarized copy must be mailed to the Financial Aid Office. Faxes, scans and emailed attachments of this document are not acceptable.
